Sinopsi:
Durante un esperimento di telepatia presso la Consec, una potente multinazionale che si occupa di armamenti, accade un incidente: il conduttore dell'esperimento muore. Il dott. Ruth rivela ai dirigenti della società l'esistenza di una organizzazione formata da Scanners, individui dotati di una potente forza telepatica. Il dott. Ruth introduce un certo Vale dell'organizzazione. Vale scopre però cose grosse: Keller, capo dei servizi di sicurezza della Consec, è un affiliato dell'organizzazione avversaria, e lo stesso Ruth è l'inventore di un farmaco, l'Ephemerol, causa di questa deformazione che crea gli Scanners. Keller uccide Ruth, diventato un pericolo; Vale uccide Keller. Lo scontro ora è inevitabile tra Vale e Revok (capo degli Scanners) in una lotta ai limiti del parossismo e del paranormale: Vale distrugge Revock, dopo aver appreso di essergli fratello ed entrambi figli del dott. Ruth.

Versioni alternative: - The death of the first scanner ('Victor Del Grande' (qv)) was filmed in two different ways: the theatrical release has Revok ('Michael Ironside' (qv)) causing his head to explode; an alternate take, featured in television versions, shows him dying of a grotesque heart attack instead. - Additional change to censored version: in the final battle between Vale and Revok, they substitute in a wimpy ending which freeze-frames Vale, shows some phony-looking blue streaking animation of a "mind transfer" from Vale to Revok, and then fades the screen, replacing the spectacular, but gory battle almost entirely.

Plot: Darryl Revok is the most powerful of all the scanners, and is the head of the underground scanner movement for world domination. Scanners have great psychic power, strong enough to control minds; they can inflict enormous pain/damage on their victims. Doctor Paul Ruth finds a scanner that Revok hasn't, and converts him to their cause - to destroy the underground movement.
